What is CHC?
CHC is free care paid for by the NHS for adults over 18.
It’s based only on your health needs — your income or savings won’t affect the decision.
CHC can pay for care at home or in a care home.

What happens in a CHC assessment?
A three-step process. We can support you at every stage.
1. Checklist
A nurse or social worker looks at 12 areas of need (called “domains”). This can feel confusing, but we’ll guide you through it so you understand what’s happening.
2. Full assessment (if needed)
If the Checklist shows you may qualify, a team of professionals (a Multi-Disciplinary Team or MDT) will do a more detailed assessment. They look at:
- How your needs affect you day-to-day
- How intense or complex they are
- How unpredictable they are
3. Decision
The team will then either:
- Recommend CHC funding, and the NHS will arrange and pay for your care, or
- Decide you’re not eligible — in which case we can support you with an appeal.

We know this is a very difficult time. We can help you request a Fast Track CHC assessment so care can be put in place quickly.
