Refer someone for advocacy

This form is for local authorities and social workers commissioning advocacy for someone they represent, usually because their own advocacy provider can’t take the case.

Please complete the following form in full in order for us to process the referral. Missing information will lead to the form being rejected.

About the person you are referring

Write N/A if they don't have one.

So we can signpost to the appropriate advocate.

Include any significant dates for support, e.g. meetings or court dates.

For example health or behavioural issues. Write None known if there is nothing.

Funding and invoicing

Usually provided by the LA finance team. Write N/A for self referrals.

If limited. Write N/A if not.

Only where the service is to be time limited. Write N/A if not.

Charges

The purchaser agrees to the charges for different elements of services provided at the rate of £42.50 per hour (excluding Continuing Healthcare Advocacy @ £85.00 per hour), as well as any additional charges for the services as agreed (Mileage).

Payment is to be made no later than 30 days after the invoice date. Interest shall be payable on the late payment of any undisputed charges properly invoiced under this agreement in accordance with the Late Payment of Commercial Debts (Interest) Act 1998.

Invoices will be sent from finance@disabilitypositive.org and will include bank account details for payment.
